Murphy's law is wrong! What can go wrong often doesn't. The problem with favorable outcomes occurs when we conclude that our actions must have been correct. But, were they really?
Rationalization is the start of a process that accepts shortcuts to safety and standard operating practices. Human factors and external pressures combine, in time, to create new norms. This process, normalization of deviation, is a large factor in many mishaps.
Expect an eye-opening discussion where we reflect on our own habits and practices!
